Boston Public Health Commission will assist consumers in enrolling in and selecting health plans, as well as retaining coverage.  They will develop quick-guides on the new marketplace application and renewal processes, and disseminate “Health Portfolios” to consumers to track health coverage, primary care providers, and Affordable Care Act information.  “Roving Navigators” will promote access to health coverage at barbershops, auto body shops, and nail salons.

Codman Square Health Center

Codman Square Health Center will conduct outreach and enrollment, hold a series of events to help attendees complete insurance applications, and provide orientations on available health coverage options.  Computers will be made available at the Codman Tech Center to access the health insurance marketplace, and consumers will be connected to the health center’s patient-centered medical home model to promote consumer engagement and self-sufficiency.  

Ecu-Health Care

Ecu-Health Care will continue its education and marketing campaign, and provide enrollment and renewal assistance on public health insurance programs.  They will train and educate applicants on navigating the health care system. 

Community Health Connections

Community Health Connections will conduct intensive street and shelter outreach to individuals who are homeless, providing eligibility determinations and application assistance.  They will hold “office hours” at local organizations to answer questions regarding programs, eligibility requirements, and sources of care in the community.  A “Community Collaborations” group consisting of health and human services organizations will be convened, serving the region’s homeless, to share information about the Affordable Care Act.  

Joint Committee for Children's Health Care in Everett

The Joint Committee for Children’s Health Care in Everett will provide application assistance and referral support, and conduct a multimedia publicity campaign to reach consumers about the Affordable Care Act.  Additional education will be provided through a multilingual helpline, website, and newsletter.  A series of educational sessions will be conducted at local community organizations on navigating the health system and using the Health Information Exchange.  

Brockton Neighborhood Health Center

Brockton Neighborhood Health Center will enroll consumers in coverage and assist with renewal applications, and provide education on the Affordable Care Act.  Community leaders from religious, cultural, and human service organizations will be convened and educated on health insurance information and available resources.

Community Health Center of Franklin County

Community Health Center of Franklin County will provide a continuum of eligibility, enrollment and review services, resource referrals to primary care, and case management.  They will educate consumers on how to navigate health coverage and care systems through computer access, including the Health Information Exchange.  Educational materials will be developed for patients to take a more active role in their health coverage and care.  

Family Health Center of Worcester

Family Health Center of Worcester will assist uninsured individuals to enroll in and maintain coverage, and connect to a primary care medical home.  They will continue weekly New Patient Orientation/Education Sessions, and expand the use of “I Speak” cards that detail useful information on the health center for patients.  Finally, they will provide individualized education and install computer kiosks for patients to use.

Fishing Partnership Health Plan

Fishing Partnership Health Plan will educate households on the Affordable Care Act, targeting “family health brokers” and community leaders to be navigators in providing frontline education and enrollment assistance.  Seminars and trainings will be offered at shore side processing plants, industry meetings, occupational safety training seminars, and settlement houses, complete with a training curriculum for all navigators.  The “Do You Love a Fisherman?” campaign will be launched with marketing materials, services, and incentives targeting spouses and family members.  

Lynn Community Health Center

Lynn Community Health Center will provide enrollment assistance and conduct monthly outreach activities at local health fairs and cultural events.  They will collaborate with health and human service providers to provide off-site outreach and enrollment services.  Monthly education sessions will be conducted on health coverage options using a Patient Navigation Guide.  

Community Action Committee of Cape Cod & Islands

Community Action Committee of Cape Cod & Islands will continue individualized “Access to Care – Navigating the Massachusetts Health Care System” educational sessions for consumers, and design a training guide identifying MassHealth mailings and navigating the complex health care system.  They will conduct a series of outreach events to educate the public on changes through the Affordable Care Act.  

Community Action of the Franklin, Hampshire and North Quabbin Regions

Community Action of the Franklin, Hampshire and North Quabbin Regions will assist eligible consumers with applications, offer education about health access services over the phone, and provide financial assistance assessments.  Referral linkages with area providers will also be enhanced.  

Caring Health Center 2012

Caring Health Center will target refugees, immigrants, and other linguistic and cultural minority groups who face barriers to enrollment and health care navigation. The health center provides comprehensive primary medical care and dental services to residents in the medically underserved city of Springfield and its surrounding areas.

Boston Public Health Commission 2012

The Boston Public Health Commission will use funds to train staff of city agencies and consumers on how to navigate the health care system and a newly launched web-centric resource database will be developed to help locate referral organizations.  Customized 'My Health Portfolios' will educate consumers on facts to consider when obtaining health care.

Whittier Street Health Center 2012

Whittier Street Health Center will provide enrollment and redetermination assistance through 'Virtual Enrollment Sites' and make referrals to services at Whittier and partner organizations. A yearly redetermination education campaign will include educational workshops and fliers/brochures based on MassHealth's guidelines.
